I need to unlink my Godaddy.com domain from my createaform page, Is this possible and how? thanks

undertheradargaming.createaforum.com need it unlinked from the three domains listed utrgaming.us, forum.utrgaming, and utrgaming.us/forum

I don't see any domains linked http://undertheradargaming.createaforum.com

You are free to repoint them.

and when I type in utrgaming.us it still sends me to the forum

You can just change in godaddy. It is not used as url.
