Internal server error while uploading @ my gallery

whenever that i tries to uplaod any file at my forum gallery especially file that reach 10mb. below are the error that i see;

The limit for gallery uploads is 12mb

but why error when uploading files with size 7mb - 12mb?

Even i wish it can be extended to 30mb.

also the 12mb is it for per upload or all uploads?
