I have changed the title of my forum but it still comes up wit hthe original name in the http// line can this be changed?

The http name can not be changed unless you buy a custom domain.

I probably did not explain properly it it the name that preceedes the  .createaforum.com currently it says patientparticipationgroups and I would like it to say patientrepresentativegroups I do not want to change the createaforum bit
Is the only way to delete this forum and start again?

You explained it properly, and CreateAForum answered it properly.

You would need to use a paid or free hosting service, when you do that, you can change it.
Also, it will remove the .createaforum from the url.
